ORIGINAL WORK CONFIRMATION STATEMENT

本人特此确认，作为“开云CRAFT创意驻留计划”的申请材料而提交的所有设计稿、草图、图像、原型、书面陈述、作品集材料以及任何其他内容，均为本人原创作品，由本人或本人品牌在本人直接创意指导下创作完成。若涉及合作作品，本人确认所有贡献者均已适当署名，且本人拥有在本申请中展示该作品所需的必要权利。

I hereby confirm that all designs, sketches, images, prototypes, written statements, portfolio materials, and any other content submitted as part of my application to the Kering CRAFT program are entirely my own original work, created by me or by my brand under my direct creative direction. Where collaborative work is included, I affirm that all contributors have been appropriately acknowledged and that I hold the rights necessary to present the work in this application.

本人理解，开云和上海时装周可能会要求提供额外信息或文件，以核实所提交作品的作者身份，任何虚假陈述、遗漏或伪造均可能导致申请资格被立即取消，若已入选则会被移除出该计划。

I understand that Kering and Shanghai Fashion Week may request additional information or documentation to verify the authorship of any submitted work and that any misrepresentation, omission, or falsification may result in immediate disqualification from the application process or removal from the program if selected.

I hereby confirm that all information provided in my application to the Kering CRAFT program—including personal details, brand information, portfolio materials, statements, documents, and any supplementary submissions—is true, complete, and accurate to the best of my knowledge.

本人知悉，无论有意或无意，任何信息遗漏、虚假陈述或伪造行为，若在后续阶段被发现，均可能导致在选拔过程中被取消资格或会被移除出该计划。本人亦知悉，开云和上海时装周可能会要求提供额外文件，以核实所提供信息的准确性。

信息使用与披露授权声明

RIGHT TO USE & DISBURSE INFORMATION STATEMENT

本人在此授权开云、上海时装周及其授权合作伙伴使用、审核、存储和处理本人为申请“开云CRAFT创意驻留计划”而提交的所有信息和材料。此授权涵盖个人资料、作品集内容、书面陈述、图像、视频以及支持文件。

I hereby grant Kering, Shanghai Fashion Week, and their authorised partners the right to use, review, store, and process all information and materials submitted as part of my application to the Kering CRAFT program. This includes personal details, portfolio content, written statements, images, videos, and supporting documents.

本人理解，上述信息可能在开云和上海时装周内部共享，但仅用于申请评估、顾问委员会审议、项目管理以及档案记录保存的目的。本人同时知悉并确认，入选材料可能被整体或部分用于与“开云CRAFT创意驻留计划”直接相关的宣传活动，包括但不限于公告和本计划的官方文件。

I understand that this information may be shared internally within Kering and Shanghai Fashion Week for the sole purposes of application assessment, advisory board deliberation, program administration, and archival record-keeping. I also acknowledge that selected materials may be used, in whole or in part, in communications relating directly to the Kering CRAFT program, including announcements and official program documentation.

本人确认此授权完全出于自愿，并知悉未经本人事先书面同意，任何提交材料均不会被用于该计划范围之外的商业用途。

I confirm that I provide this permission freely and acknowledge that no submitted materials will be used for commercial purposes outside the scope of the program without my prior written consent.
